A qualified supervisor must be in good standing with their employer and all regulatory bodies (e.g., state board of pharmacy) that have jurisdiction over their worksite.
The supervisor must also have at least 5 years of experience working directly with, or supervising compounded sterile preparations (CSPs) production.
If the supervisor has less than five (5) years, they must attach a letter from the pharmacy director describing their qualifications to supervise CSP production.
I am completing the Competency Attestation Form. Who is considered a qualified supervisor?
