51 STEPS
1. Visit PTCB.org and click LOGIN.
2. Type your username.
3. Type your password.
4. Click Login.
5. Scroll down to Earn Your CPhT Certification. Then click APPLY NOW.
6. Review the Candidate Agreement. Click the checkbox next to "I agree to the Candidate Agreement above".
7. Click Continue.
8. Click Pathway 2
9. Click Continue
10. Click Add Work Experience
11. Start typing Employer's name
12. Some employers will appear in the dropdown selection. Major chains may autofill an address, others will require it to be typed in.
13. Scroll down and type in Supervisor's name.
14. Select Month, day and year from the dropdowns.
15. Select Month, day and year from the dropdowns.
16. Select Month
17. Select day.
18. Select year.
19. Select Full-Time or Part-Time.
20. Click Current Employer if they are your current employer.
21. Scroll down and click Save
22. If you need to add additional work experience, click Add Work Experience.
If you are finished entering your work experience, click Continue.
23. Review the knowledge attestations.
Scroll through each knowledge statement list to review the individual knowledge statements.
Select the checkbox to attest to the required knowledge.
24. Note: the checkbox will NOT become available to select until you scroll through the knowledge statement lists.
If you cannot select the checkbox, return to the list of knowledge statements and scroll to the bottom, then try again.
25. Describe the preparation activities you completed for each knowledge domain.
26. Repeat for each knowledge domain.
27. Click Continue.
28. Repeat the attestation process for each knowledge domain.
29. Repeat the attestation process for each knowledge domain.
30. Click Continue.
31. Repeat the attestation process for each knowledge domain.
32. Repeat the attestation process for each knowledge domain.
33. Click Continue.
34. Select the option that describes you.
35. Click Continue.
36. Select if you do or do not require testing accommodations.
37. If you require testing accommodations, download the Testing Accommodations Request From, then select Yes, I require testing accommodations.
The Testing Accommodations Request From can be submitted to PTCB at ptcb.org/help or by fax at (202) 888-1699.
38. Click Continue.
39. Type your electronic signature.
40. Click Continue.
41. Answer the sponsorship questions. Click Continue.
42. If you are sponsored, select Yes to "Is your employer or educator paying for your certification fee at this time?"
43. Select your sponsor organization from the drop-down menu.
44. If your sponsor has provided you with a voucher, enter your voucher number and any other requested information (e.g., employee ID).
45. Click Continue.
46. Review the Sponsor Terms and Conditions. Select the checkbox next to "I agree to the terms and conditions above."
47. Click Continue.
48. Once your application is submitted, a Sponsorship Confirmation page will be displayed.
Click the PTCB Account homepage link to continue.
49. If you are not sponsored, you will be taken to the Initial Certification Application Summary. Review and click Add to Cart.
50. Click Checkout.
51. Add credit card information and click Continue
