Past misconduct does not automatically disqualify you from earning PTCB credentials. PTCB will review each case individually. Note: Apply only after your misconduct case is resolved with the associated court, government agency, State Board of Pharmacy, credentialing organization, or professional association.
When you apply for a PTCB credential, follow these steps if you have past misconduct to disclose:

- Fill out the PTCB Misconduct Disclosure Checklist Form.

Submit Official Documentation
- Official documentation must include all related information issued by a court, government agency, professional association, or credentialing organization that explains how the situation was handled and resolved.
- Applications without this documentation will not be reviewed.

Eligibility Review
- Past misconduct does not automatically disqualify you.
- PTCB reviews each case individually and will provide a written decision within four weeks.

Pay the Application Fee
- Your application and fee must be submitted before PTCB reviews your eligibility.
