CPhT reinstatement candidates with returned applications: If your application has been returned, you must provide documentation of the required CE activities completed during your previous recertification cycle and up until your reinstatement application date. Your application resubmission is due within 30 days of your reinstatement application return.
If you cannot provide documentation of the required CE activities completed during your previous certification cycle and up until their reinstatement application date, then you have not met the required CEs you attested to completing when applying and you are not eligible to re-submit your application. To continue, you will need to submit a new application for CPhT reinstatement by your reinstatement deadline (one year following certification expiration date).
If you are past your reinstatement deadline, then you must apply for CPhT Certification and retake and pass the Pharmacy Technician Certification Exam (PTCE).
