What are the requirements for proof of completion for a PTCB-Recognized Education/Training Program?

Proof of Completion for PTCB-Recognized Education/Training Programs (REP) for CPhT or CSPT Certifications

Acceptable documentation includes the following:

  • Certificate of completion that includes: 
    • Your name
    • Education/training program title
    • Completion date
  • Letter from an education/training provider on official letterhead that includes: 
    • Your name
    • Education/training program title
    • Completion date
  • Unofficial transcript or diploma

Unacceptable documentation includes the following:

  • Exam score reports issued by PTCB

Note: Proof of program completion is only accepted after successful completion of the program. Letters sent in advance of the program completion date will not be reviewed or accepted by PTCB. The documentation must be received by PTCB within 60 days of your exam date. Find out more about your options here if you're about to graduate from a REP but haven't yet done so.

Proof of Completion for PTCB-Recognized Education/Training Programs for Assessment-Based Certificates

Assessment-Based Certificate program candidates must upload proof of completion at the time of application.

Before submitting your proof of completion, you must ensure the documentation meets the below requirements. To submit your proof of completion documentation, you must submit a help request and provide your attachment(s) using the help request form.

Certificate of Completion

  • Your name
  • Program title
  • Provider name
  • Completion date
  • # of hours awarded, if applicable 
  • ACPE Universal Activity Number (UAN), if applicable
