What are the requirements for proof of completion for a PTCB-Recognized Education/Training Program?

Submission Timeline:

CPhT or CSPT Certifications: Proof of program completion is only accepted after completing the PTCB-Recognized Education/Training Program (REP). Documents sent in advance of the REP completion date are not accepted by PTCB. The documentation must be received by PTCB within 60 days of submitting your application.

Assessment-Based Certificates: Assessment-Based Certificate candidates must upload proof of program completion document into the application.

Proof of Completion Requirements:

Acceptable documents:

  • Certificate of completion that includes: 
    • Your name
    • Education/training program title
    • Completion date
  • Letter from an education/training provider on official letterhead that includes: 
    • Your name
    • Education/training program title
    • Completion date
  • Unofficial transcript or diploma

Unacceptable documents:

  • Exam score reports issued by PTCB
  • Documentation that does not match the application (Different educational institutions, different name)
  • Missing criteria from the above list

  • Provider name
  • Completion date
  • # of hours awarded, if applicable 
  • ACPE Universal Activity Number (UAN), if applicable
