Individuals, groups, or organizations who want to submit a conduct complaint against a PTCB credential holder or candidate must complete the Conduct Charge Statement, and send the completed form by submitting a Help Request or fax (202) 888-1699 to ATTN: PTCB Credentialing Operations, Conduct Charge Statement.
